Platinum package - $200/month
Consider this as meeting all your primary care and all your GYN needs in one price. With our focus on wellness, this package is intended to help you start living your best life.
Includes GYN well woman care, annual screening ultrasound, colposcopy if needed and minor office procedures.
Includes management of all your office health care needs. Got a cut, don’t call the ER, we’ll take care of it. If avoiding going to urgent care is a top priority, then this plan will be as well.
Includes preventive care labs and EKG
Includes 12 IV therapy appointments and Wellness counseling.

You get everything in the platinum package as well as HRT labs as needed for workup or for management of those requiring hormone replacement therapy.
Gold package - $150/month
This package is ideal for people who have a primary care doctor, but want all their gynecologic needs covered for a known price. This includes the focus on wellness and also provides you instant access and maintains our wellness focus.
Includes GYN well woman care, annual screening ultrasound, colposcopy if needed and minor office procedures.
Includes 6 IV therapy appointments and Wellness counseling.
20% off any noncovered services such as additional IV therapy offered at Advanced GYN and Wellness and Advanced Aesthetics and Wellness. (Excludes Anesthesia and surgery center fees as those are not within our control.)

Gold/HRT package- $165/month
You get everything in the platinum package as well as HRT labs as needed for workup or for management of those requiring hormone replacement therapy.
Postpartum Package - $2500 ($1000 if pt has an annual package) - only available for 1 patient/month to ensure provider availability
1 hour house call daily x 5 days after you are home from the hospital - can be spent on you or simply holding the baby while you have time to eat a meal or take a shower

Screening phone call at 2-3 weeks and management or referral as necessary.

This package is ideal for people who are in that perimenopausal/postmenopausal phase of life and just want to feel normal again as well as get their annual gyn care take care of and have all of it handled by a women’s health professional who doesn’t make any money selling your expensive HRT products.
Covers full initial HRT workup and labs.

Includes HRT monitoring and monitoring labwork for people taking compounded HRT.

HRT Packages do NOT cover medications - compounded products are produced and sold at a compounding pharmacy and commercial medications can be provided at cost via the office pharmacy or purchased from your preferred local pharmacy.
Nonsubcription (Fee for Service) Office Services:
New Patient - $300 (all previous patients of Dr. Moore will receive a $50 discount)
Return Patient - $250
TVUS - $150 (if ultrasound only services and not part of a visit)
IUD removal - $50 (if done as a stand alone visit. Included in the visit fee if being see for a full appointment.)
IUD insertion $250 + cost of IUD
Freezing/skin tag removal - $250 up to 15 lesions.
Colposcopy - $350 plus pathology cost
Endometrial biopsy - $100 if at a stand alone visit - included if part of a visit. Pathology will be at cost.
Bladder instillation - $200 or $600 for 4. (usually done weekly, but schedule can be adjusted or they can be performed as needed for IC flares)
Ovulation Induction Cycle - $300 - doesn’t include medications. Can be purchased at cost through the office.
Confirmation of Pregnancy - $400 (included in Gold and Platinum plan and will pause plan for the duration of pregnancy.)

IV Therapy Options
IV therapy - The following should be viewed not as a “menu” to order off of, but information about the costs of managing different things. Every patient will have an IV therapy consultation where we discuss the goals of IV therapy/patient’s health goals, perform appropriate nutrient testing and decide together on a treatment plan. Sometimes this is easy - you have a GI bug from your kid - hydrate and nausea medicine. However, sometimes, patient’s have more complex needs that may involve changing the IV therapy at every appointment. When patients are canddiates for repetitive therapy or on a plan with multiple components, we desire to provide a cost savings because we don’t have to start from scratch each visit.
Some IVs include Hydration therapy like the Myers and Hangover, but others can have a liter of IV fluids provided for an even bigger pick me up. Adding a liter of normal saline or lactated ringers is $35. This covers both the supply cost as well as the extra time it takes for the IV to infuse.

Office Procedures (the anesthesia portion of each is $500 and is nonnegotiable and nondiscountable - that is purely what it costs to provide safe and effective care and cover the costs of extensive government regulations over and above what is recommended by the American Society of Anesthesiologists (ASA))

Surgery Center (Hospital) Hysterectomy - Prices are based on uterine weight as this is the key determining factor in how long the procedure takes. Price includes bilateral salpingectomy for those removing the tubes and saving the ovaries or removal of tubes and ovaries for those who require oophorectomy or who are over 55 (and postmenopausal) and desire removal. Most uteruses without fibroids are <250 gm. We can give you a very close size estimate based on the ultrasound measurements.

* All prices in this section are for provider services only. These do not include the fees charged by the facility where the procedure is performed. Those fees are set by the facilities and go exclusively to the facility. We desire to make keep surgical care affordable to our providers therefore we are partnering with a local surgery center to provide access to reasonably priced surgical services. Surgery Centers are highly regulated and provide excellent care. They are not allowed to cut corners and are accredited by the same organizations that accredit hospitals. They aren’t cheaper because they provide less good care. They are reasonably priced for planned outpatient procedures because that’s all they do. These services are equivalent to hospital OR care, but due to a combination of factors, they cannot provide services to the highest risk patients. There are some patients who because of health factors or because of the complexity or nature of the surgery, would need to have their care provided in a hospital setting. We will work with patient’s who require hospital care to get their procedures approved by their insurance as most hospitals require this for any nonurgent procedure. While hospitals can provide excellent care, they do so at a higher cost (they take care of more complex patients and have things like blood banks that explain the value of that cost increase) and with less individual convenience. None of this means hospitals are bad, we just feel that hospital care is best reserved for those who need it and by moving things that don’t require hospital care our of hospital, we make more space for those who require those services. It’s the same with other hospital services like ERs - wait times are longer becasue people are seeking nonemergent care - often becasue they can’t get an appointment with their doctor. Lets all fix this system together.
